The Home Care Nurse provides high-quality nursing care to clients in their homes, ensuring their medical needs are met while promoting independence and well-being.
The role involves traveling to clients’ homes, assessing their health conditions, administering treatments, providing education, and collaborating with healthcare professionals and family members to ensure comprehensive care.
Key Responsibilities
Patient Care & Nursing Services:
Provide personalized, one-on-one nursing care to clients in their homes, ensuring adherence to care plans developed by healthcare providers.
Administer medications, treatments, and other medical interventions, following physician orders and nursing protocols.
Conduct regular assessments of clients' health status, including vital signs, physical conditions, and responses to treatments.
Perform wound care, catheter care, injections, IV therapy, and other clinical procedures as required.
Health Monitoring & Documentation:
Monitor changes in clients’ health and report any significant changes to the supervising nurse or physician.
Maintain accurate, up-to-date clinical documentation, including assessments, treatment notes, and medication administration records.
Ensure compliance with health regulations and care home policies regarding record-keeping and reporting.
Care Coordination & Family Education:
Collaborate with other healthcare providers, such as doctors, physical therapists, and social workers, to develop and adjust care plans.
Educate clients and their families about managing chronic conditions, medications, and home safety measures.
Provide emotional support to clients and families, helping them navigate healthcare challenges and fostering a positive caregiving environment.
Emergency Response & Critical Care:
Respond promptly to emergencies and provide appropriate medical interventions, including first aid or emergency procedures.
Coordinate with hospitals or emergency services when a higher level of care is needed.
Health Education & Preventive Care:
Provide health education to clients and their families, emphasizing preventive care, proper nutrition, and healthy living habits.
Encourage adherence to prescribed treatment plans, including medication schedules, rehabilitation exercises, and follow-up appointments.
Quality & Compliance:
Ensure all nursing activities are conducted in compliance with the care home’s policies, healthcare regulations, and best practices.
Participate in quality assurance programs and contribute to continuous improvement initiatives aimed at enhancing patient care.
Qualifications & Experience
Minimum of 5 years of nursing experience, ideally in home care, geriatrics, community nursing, or related fields.
Bachelor's Degree in Nursing (B.Sc. Nursing) or equivalent nursing qualification.
Valid nursing license and registration with the relevant nursing council.
Valid driver’s license is required.
Experience in wound care, medication management, and home-based medical care is an advantage.
CPR and First Aid certifications are desirable.
